Transcript Requests

  • Former Students:
    Former students who have graduated PRIOR to the current year, please fill out the Former Student Transcript Request Form (see word document at bottom of page). The transcript request form should be filled out completely, signed, emailed, faxed or dropped off to the Student Support Center.

    If you are in need of immunization or medical records you must contact the Health Office at 716-836-7200 extension 8330

    Current Students:
    Current students can stop down to the Student Support Center and request a Transcript Request form from either their School Counselor or Mrs. McIntosh, the SSC Secretary.

    OR

    Current students can print out the CURRENT Student Transcript Request below and bring it to the SSC and drop it off to their School Counselor or Mrs. McIntosh
